Family ID Athletic Registration

The Whitesboro Central School District Athletic Department offers online athletic registration through FamilyID.

FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for our athletic programs.

During the registration process, you can select a sport at an appropriate level for your child, and complete all required forms including:

• Health History Update, Dominick Murray Cardiac Prevention Act, Guidelines for Interscholastic Athletics, Procedures for Determining Academic Eligibility at the Middle School and High School, Parent Permission Form, Student Accident Insurance Plan, Emergency Contact Card Information.
• NY State School Health Examination Form and Authorization for Administration of Medication (can be uploaded through FamilyID or submitted directly to the Nurse’s office at your child’s school).

Please note that only one Family ID account is required to register one or more children from the same family. To register more than one child, choose “add new participant” once you have completed the first registration for the first child.
