    WHITESBORO CENTRAL SCHOOL DISTRICT SCHOOL PHYSICAL EXAM REQUIREMENTS

    • The State Education Law requires medical examinations of school children at specific grade
      levels.
      “A health certificate shall be furnished by each pupil in the public schools upon his/her entrance
      in such schools and by each child entering kindergarten, first, third, fifth, seventh, ninth and
      eleventh grade thereof. An examination of any child may be required by the local school
      authorities at any time in their discretion to promote the educational interests of such child.” 
    • Ideally, the family physician should perform the medical examination since he or she is familiar
      with your child’s family, home, previous illnesses, immunization status and other background
      factors that assist in evaluating the total health status of a child.
    •  In order to provide you with the necessary time to have a medical examination, we are providing
      you with this health examination form, which can be completed by your family physician and
      returned to the health office in your child’s school.
    •  If the completed form is not received at the school, we will assume that you desire the medical
      examination of your child to be given by our school nurse practitioner. The cost of the medical
      examination given by your family physician is your responsibility while that of the school nurse
      practitioner is paid from the school budget.
