Home Delivery Consent Form for Student Meals

A home delivery consent form was mailed out on Monday, August 31 from the Food Service Department. This consent form is for families whose child(ren) are attending school on the 100% remote learning model. We are unable to deliver meals to your residence UNTIL this consent form is filled out and signed by a parent/guardian and returned to the Food Service office. Your prompt response is greatly appreciated!
